Background of the Study

Home-based care has become an essential component of healthcare delivery, particularly for patients with chronic conditions such as heart disease. Home-based cardiac nursing interventions include regular monitoring of vital signs, medication management, lifestyle education, emotional support, and rehabilitation, which have been shown to improve patient outcomes in managing chronic cardiac conditions (Adeoye et al., 2023). In Katsina State, heart disease remains a major public health issue, with many patients requiring long-term care and management. However, the burden on healthcare facilities is often overwhelming, leading to a growing emphasis on community-based care models (Khan & Adebayo, 2024).

Cardiac nurses involved in home-based care are uniquely positioned to provide personalized care that aligns with the patient's specific needs, contributing to better disease management, improved adherence to medications, and reduced hospital readmissions (Umar et al., 2023). Despite the growing recognition of the importance of home-based interventions, limited research has been conducted on the effectiveness of these interventions in improving cardiac patient outcomes in Katsina State. This study will explore the role of home-based cardiac nursing interventions in enhancing patient outcomes, focusing on factors such as patient satisfaction, adherence to treatment, and quality of life.

Statement of the Problem

Heart disease management requires continuous and personalized care, yet many patients in Katsina State lack adequate access to hospital-based services, particularly in rural areas. Home-based cardiac nursing interventions offer a promising alternative to improve patient outcomes. However, there is limited research on the effectiveness of these interventions in the region, and a need to assess their impact on patient outcomes. This study will investigate the role of home-based cardiac nursing interventions in improving patient care outcomes in Katsina State, particularly in relation to medication adherence, symptom management, and overall quality of life.

Objectives of the Study

1. To assess the impact of home-based cardiac nursing interventions on the adherence to treatment and medications among cardiac patients in Katsina State.

2. To evaluate the effect of home-based nursing care on the quality of life of cardiac patients in Katsina State.

3. To explore the experiences and challenges faced by both cardiac nurses and patients in home-based care settings.

Research Questions

1. How do home-based cardiac nursing interventions impact medication adherence and treatment outcomes among cardiac patients in Katsina State?

2. What is the effect of home-based nursing interventions on the quality of life of cardiac patients in Katsina State?

3. What challenges do home-based cardiac nurses face in delivering care to patients in Katsina State?

Research Hypotheses

1. Home-based cardiac nursing interventions significantly improve medication adherence and treatment outcomes among patients in Katsina State.

2. Home-based nursing care leads to a significant improvement in the quality of life of cardiac patients.

3. There are significant challenges faced by home-based cardiac nurses in Katsina State that may limit the effectiveness of their interventions.

Scope and Limitations of the Study

This study will focus on home-based cardiac nursing interventions in Katsina State, evaluating their impact on patient adherence, quality of life, and overall health outcomes. It will be limited to patients with chronic heart conditions who are receiving home-based care. Limitations include potential biases in self-reported patient outcomes and the small sample size in rural settings, which may limit generalizability.

1. Home-Based Cardiac Nursing Interventions: Nursing interventions provided to cardiac patients in their homes, including monitoring, education, and support to manage their condition.

2. Adherence to Treatment: The extent to which patients follow prescribed treatment plans, including medication and lifestyle changes.

3. Quality of Life: A measure of an individual’s well-being and ability to perform normal activities in the context of living with a chronic illness like heart disease.
